    Take out your telescope and look at the moon. Simply put: Waxing cresent: go to G7 (Five-star isles). First quarter: G3 (Star belt Archipalago). Waxing gibbou: B4 (Greatfish isle). Full: E1 (Cresent moon island). Waning gibbous: A6 (Diamond steppe island). Last quarter: F5 (Bomb island) Waning cresent: C2 (Spectacle island). This is also the pattern in the ghost ship chart when you get it.

    Wrong information. Go to Diamond Steppe Island (A6), use the hookshot to climb your way to the hole on top, then enter the warp pot in front of you, then the one by the floor master, then go through a gap in the boat railing and enter the warp pot directly below it. After beating the floor master, walk to forward, and enter the pot to the left of the torch. You'll then find the ghost ship chart.
