1. Up, A, Down, A, Left, A, and Right at the Title Screen (Set to international mode first) 2. Choose a level (Only if there's level select) 3. Hold Y until the level appears.

You can do it on Japan mode too, but the button code is different. The actual order of when you press the A button (Genesis C button) doesn't actually matter in any version, so for me the easiest in the Japanese game is to hit A, A, A, Up, Down, Down, Down, Left, Right. You still have to hold Y as Tom mentions.

I tried both the Japanese and international versions I do the combination and I hear a ring sound effect I press X to get to level select and hold Y until the level appears but nothing happened.
