    That could possibly be due to many factors; Low level, low skill level, a weak hammer tool, low Luck stat, low Vitality and/or Focus stat(s), not completing the crafting process quick enough... Getting Top Quality can be quite frustrating and it's kinda random when it gets down to it. Maybe you should consider taking a break and doing something else for a bit?
