I was doing missions i didnt get 3 stars on in the fen pack...and i missed 1 epic box. I was close to finishing when a door spawned one of those explosive guys, i sent piper up and shot him. I forgot when people walk they can be hurt by things so when i shot him piper died halfway up the ladder...

    I despise those exploding enemies, luckily those don't appear later levels especially in Fen's DLC later levels.
