You get that sometimes when you're using a weapon that has the "Slots+" ability. The "Weapon Slots" mixtures from the Apothecary in the bazaar increases your chances of getting extra slots, too. On a level 3 weapon, you can get up to 8 slots!

oh yeah I knew that, I just thought the maximum for a lv 2 weapon was 5

I'm not sure there IS a maximum. I'm pretty sure I once got a level 1 weapon that had 5 slots!
