    Yes, it's from the American version. I've only seen it on Hell difficulty though.

    I don't think I've seen any other new ones on Hell yet. I'm pretty sure several were added in general with the DLC though.

    I actually know where Old Man is now. The in game picture is incomplete. Near the back of the official Japanese guide, you can find the full image. Old Man is off to the right laying on his side on the ground. It looks like he couldn't keep up and collapsed. He shouldn't have run in full armor.
