You walk to the ledge above, as if you were entering Forest Haven. When you first use your grappling hook, stop your swing and lower yourself further down on the rope. Adjust your angle so you're facing the waterfall, then start swinging again. When your swinging slightly into the waterfall, let go, and you should leap through the water and into the small cave beyond.
