    They will give you special presents for certain occasions, and at 5+ hearts they can trigger unique cutscenes. Besides that I think they just get a scarf/bow and are stuck living in your house unless you unregister it. There is a celebration for the date it became your furmiliar.

    If it's a herding pet and the barns etc are on your main farm area it will still go outside :]
