You must get a Moon's Tear. Then, trade it in to the Business Scrub in South Clock Town for the Land Title Deed. Afterwards, head to the Stock Pot Inn and find the "hand" or ??? and give him the Land Title Deed as toilet paper to get the Heart Piece.
