Have you also tried to set the level difficulty to Hard Mode? That may be the main issue here since all of the second skulltula's for the Story Scenarios require the difficulty to be set to Hard Mode. After you change the difficulty to Hard and meet the requirements (while using Ganondorf) the skulltula should appear in the Mountain Keep
